Public Records Request

  • The Washington Township Health Care District (the “District”) generally requires all requests for public records under the Public Records Act (the “Act”), California Government Code § 6250 et seq., to be submitted to the District via the form below on this web page.
  • This policy exists to ensure that your request is routed to the correct individual so that it can be processed in accordance with the requirements of the Act. If you attempt to submit a request for public records by any other means, including, but not limited to, a letter or email to a District staff member, your request may not be routed correctly, and accordingly, your request will not be considered received under the Act.
  • The District reserves the right to charge for records in accordance with the Act.
